The following outlines the Terms and Conditions of the M-Care Medical Clinic Membership Loyalty Programme. By completing and submitting this application form, you acknowledge that
you have read, understood and agreed to all the terms and conditions stated herein.

Eligibility and Participation:
• Only registered patients of M-Care Medical Clinic are eligible to be enrolled in the Programme.
• Participation is voluntary, and patients may opt out at any time.

Complementary Consultation:
• After five consultations, eligible patients receive one complimentary consultation (Telemedicine Consultation, Results Review Consultation, or General Consultation).
- Exclusions:
• This Complementary Consultation does not include medical procedures, medications, lab tests, or physiotherapy services.
• Cards have no cash value.
- Lost Cards:
• Lost cards incur a replacement fee.
- Programme Amendments:
• M-Care Medical Clinic reserves the right to modify or terminate the Programme without notice.
- Family Membership:
• Immediate family members receive individual cards, with points accumulated and benefits shared as a family.
- Transferability:
• Membership cards are non-transferable, cards are for individual or authorized family use only.
- Affinity Partners:
• Benefits offered by select Affinity Partners are at their discretion and may change without notice.
- Governing Law:
• These terms are governed by the laws of St Lucia and disputes subject to the jurisdiction of the courts in St Lucia.
